Before Your Visit
If you are a new patient or if it has been awhile since your last visit, you can expect to receive a welcome packet with information about our offices, providers, and policies. Please be sure to complete the Patient History Form to the best of your ability and mail it to our office:

When You Arrive
When you arrive at the office, you need to have the following items with you to check in:
- your driver's license or ID card
- your insurance card
- a referral from your PCP (if you have an Oxford health plan)
- your co-pay, which is due at check in (cash, check, Visa or Mastercard)
- your completed patient history form
After you check in at reception, your physician's Clinical Assistant greets you, leads you into an exam room, and charts all of the information that your physician needs for your consultation.
If You Need To Cancel An Appointment
Occasionally, something unavoidable comes up that makes it impossible for you to keep an appointment with us. Please note that we require 24-hour notice to cancel an appointment. This allows us to offer your appointment slot to someone who may be waiting to be seen urgently.
Patients who miss an appointment (or cancel an appointment with less than 24-hours notice) will be charged a $50 missed appointment fee, and patients who repeatedly miss appointments will be referred out of our practice.
